Reminders for Managing Psoriatic Arthritis: Medication and Daily Routine

Psoriatic arthritis management spans joints and skin simultaneously. Consistent medication and daily care routines are easier to maintain with phone-call reminders.

Psoriatic arthritis (PsA) is a chronic inflammatory condition affecting both the joints and skin of people with psoriasis. It requires management of two simultaneous chronic conditions — inflammatory arthritis and psoriatic skin disease — with overlapping but distinct treatment approaches. Medication consistency, regular monitoring, and daily skin care routines all require reliable daily prompts to maintain over the long term.

DMARD and Biologic Medication Timing

Like rheumatoid arthritis, PsA is treated with DMARDs and biologics. Methotrexate (weekly), leflunomide (daily), and apremilast (twice daily) each require consistent dosing. Biologics — adalimumab, secukinumab, ixekizumab, ustekinumab, guselkumab — are self-injected at intervals ranging from fortnightly to every 12 weeks.

Apremilast (Otezla), a PDE4 inhibitor often used in PsA, requires twice-daily dosing with a specific titration schedule when starting — the dose is increased every few days over the first two weeks. A reminder that names the current dose during titration prevents confusion: 'Morning apremilast: Day 6 titration — take 20mg now with food.'

Biologic injection day reminders — including preparation instructions (remove from fridge 30 minutes before injection, check expiry date) — make the process reliable even on busy days.

Skin Care Routine Reminders

Psoriatic skin disease requires a consistent topical treatment routine: emollient application (often twice daily), topical corticosteroids or vitamin D analogues on active plaques, and scalp treatments where relevant. The morning application before dressing and the evening application before bed are the two fixed points that daily reminder calls can anchor.

For patients using phototherapy (narrowband UVB) two to three times weekly, a session reminder on treatment days — 'UVB phototherapy today — remember your appointment and remove any creams from the treated areas beforehand' — prevents missed sessions that slow progress.

Calcipotriol (vitamin D analogue) and potent topical steroids have maximum usage periods to prevent thinning. A reminder to review topical treatment duration with a dermatologist when a course approaches its limit prevents inadvertent overuse.

Monitoring and Appointment Structure

PsA monitoring includes joint disease activity assessment, skin PASI scoring, blood tests for DMARD safety monitoring, and cardiovascular risk assessment (PsA carries elevated CV risk). These appointments and tests need to happen regularly and on time.
