Fit Notes: the Seven-Day Deadline People Miscount
Self-certification covers 7 days — calendar days, including weekends, which is where people miscount. Nurses and pharmacists can issue fit notes since 2022, and you never need a note to return to work early.

Being ill is one of the few situations where the admin gets harder as you get less capable of doing it. There is a deadline in the middle of it that a lot of people miscount, and three rules about fit notes that employers get wrong often enough to be worth knowing by heart.
When do you actually need a fit note?
After seven days — and the seven days are calendar days, not working days. GOV.UK states it directly: "if employees are off work for 7 days or less, they do not need to give their employer a fit note or other proof of sickness from a healthcare professional."
The sentence that follows is the one people miss: "this includes non-working days, such as weekends and bank holidays."
So the clock does not pause for a Saturday. Fall ill on a Thursday and by the following Thursday you are already at day eight, having been at work for only three of them. People who count working days routinely arrive at the seventh working day — nearly a fortnight in — and find they have been unable to prove absence for several days they assumed were covered.
Below that threshold you self-certify: you tell your employer you were ill, usually on a form, and no healthcare professional is involved.
Who can issue one?
Not just a GP, and this changed several years ago without most people noticing. The Department for Work and Pensions amended the legislation "with effect from 1 July 2022 to enable nurses, occupational therapists, pharmacists and physiotherapists to certify fit notes in addition to doctors."
That matters when a GP appointment is two weeks away and the deadline is Thursday. If you are already under the care of a physiotherapist for the back problem keeping you off work, or seeing a nurse regularly, the note may be obtainable from someone you are seeing anyway.
A second change, from April 2022, removed "the requirement for signatures in ink" — which is what made digital fit notes possible. A fit note that arrives as a PDF with no handwritten signature is not a defective one, and an employer refusing it on that basis is working from an out-of-date understanding.
Do you need a note to go back to work?
No. This is the rule most often got wrong, and getting it wrong costs people days of pay and their employers days of work.
GOV.UK is unambiguous: "you can go back to work at any time you feel able to (including before the end of the fit note) without going back to see your healthcare professional."
There is no such thing as a required "return to work note" in this system. If your fit note says four weeks and you feel able after two, you can return after two, and nobody needs to re-certify you. Plenty of workplaces believe otherwise and ask for a note that does not exist.
What does "may be fit for work" actually mean?
It means the healthcare professional thinks you could work with some adjustment — a phased return, altered hours, amended duties — and has written that as advice. It is a suggestion, not an instruction.
GOV.UK puts the status of those options plainly: "these options are not binding on you, and you should feel free to discuss other options."
That cuts both ways, and it is worth understanding before a difficult conversation. You are not obliged to accept precisely what is written, and your employer is not obliged to provide precisely what is written either. It is a starting point for a discussion about what would let you work, not a prescription for your manager to follow or ignore.
If that discussion goes badly, or there is a dispute about sick pay, ACAS and trade unions exist for exactly this and a blog does not.
Which reminders actually help?
Two, and the first is the one that catches people:
- Day five of any absence. Not day seven. Two days of warning is enough to get an appointment or a pharmacy conversation before the threshold, and it costs nothing if you are back at work by then. Set it on the first day you go off sick, while you still have the presence of mind.
- Two days before a fit note expires, if you are on a longer absence. Notes run out, and a gap in certification is a gap in evidence for sick pay.
Both are one-off reminders rather than recurring ones, and both should be set at the start when you are least motivated to think about admin — which is exactly why a reminder is the right tool rather than an intention.

Where this applies
These are the rules for employees in England, Scotland and Wales, published by GOV.UK. Northern Ireland has its own arrangements, and self-employed people are in a different position entirely.
Your employer may also have its own sickness policy requiring earlier notification — that is contractual and sits on top of the statutory picture rather than replacing it. Check what your contract says about telling someone on day one, because that is a separate obligation from the fit note and is the more common reason people get into difficulty.
